What is a Form 5?

A Form 5 is a Certificate of Electrical Compliance under the Health (Public Buildings) Regulations 1992 in Western Australia. It certifies that the electrical installation at a public building or public event is compliant, and it covers temporary installations such as the power for an outdoor cinema, a market or a festival. It can only be completed by a licensed electrician.

Do you need one?

Usually only if your event is open to the public and your council asks for it. A private backyard party does not normally need one. Your council's event application pack is the final word, so check it when you apply. The electrician decides whether the installation can be certified. This page is general information, not legal advice.

How it works

How we arrange your Form 5

Tell us when you book that your council needs a Form 5, and when it wants it.
We arrange the electrician. A licensed electrician is booked for your event, subject to availability, so you do not need to find one.
The electrician comes to site and looks over the whole electrical system, including our generator or battery inverter and everything connected to it.
They sign the certificate. Under WA Health guidance the electrical contractor submits the Form 5 to the local government, and we confirm the lodging step with you when you book.
The fee is $500, added to your hire. It is only charged when you need a Form 5.

Council rules

Check your council early

Each council runs its own event approval process, and the rules differ on timing and on generators. The WA Department of Health explains the electrical requirements for public buildings on its electrical and lighting requirements page. WA Health discourages single phase generators of 10 kVA or less at events, and some councils add rules of their own, so ask your council's environmental health team which generator rules apply before you finalise your power plan.

Our Terms and Conditions (clause 20) set out who is responsible for council approvals. If a Form 5 is required, it is one of the approvals the hirer arranges, and we make that easy by organising the electrician.

Councils differ. Some want it before the event, others on the day or within a short time afterwards. Check your council's event requirements and let us know when you book so the electrician is lined up for the right time.
No. Test and tag, to AS/NZS 3760, checks individual leads and equipment. A Form 5 certifies the electrical installation at the event as a whole. We test and tag our equipment either way.
The electrician looks over the whole electrical system on site, so tell us who else is plugging in, such as food trucks, stalls and a bar. The more we know when you book, the smoother the inspection.
